Clinical Care Coordination is for mob with complex mental health issues who have extra needs and aren’t getting enough support elsewhere.
What is Clinical Care Coordination?
Through this service we can support you to:
Work with other health professionals
This means talking and meeting with doctors, psychiatrists, and other therapists to make sure you get the right care.
Create a support plan
This involves helping to plan your care and working with other services, like Personal Helpers and Mentors, to give you all the support you need.
Focus on your well-being
This means building a good relationship with you, checking on your mental health regularly, and helping them manage both your mental and physical health.
Who can we help through Clinical Care Coordination?
We can support Aboriginal and Torres Strait Islander people 18 years or older in the metropolitan area of Adelaide, who have complex mental health needs.
