Nunkuwarrin Yunti provides a care coordination service to support clients to manage their chronic condition. We have a Chronic Conditions Management Team, managed by the Chronic Conditions Management Coordinator. The team includes an Aboriginal Health Practitioner and Aboriginal Health Worker and supports clients to understand their health needs in order to make healthy changes to their lifestyle to help minimise the impact of their chronic disease on their health and wellbeing.
The team works with clients to develop their own wellness care plan, together with a GP and visiting specialists and allied health workers. We are then available to discuss with clients their progress and any other needs they may have to help them manage their chronic condition, including access to medications and support by pharmacists.
The Team provides general health checks and immunisations, and can arrange and support clients to access other services in Nunkuwarrin Yunti such as Harm Minimisation (drugs and alcohol), Tackling Tobacco, psychology and & counselling and the Health Lifestyle program.
The Chronic Conditions Management Team can also arrange appointments and follow up with our visiting specialists (renal, ENT, endocrinology and infectious diseases specialists) and allied health professionals (physio, diabetes educator, podiatrist, psychologists) who can assist clients to maintain healthy lifestyles.
The team includes an Aboriginal Health Practitioner who’s role is to provide targeted support services for Aboriginal people at risk of or living with a blood borne viral (BBV) illness (Hepatitis B, Hepatitis C and HIV/AIDS) and sexually transmitted infections (STI), and their families as needed.
All services are bulk billed, and we aim to ensure that outside service providers that clients might be referred to also bulk bill. The Team is available through appointment, by referral from a GP and when available, can also be accessed by clients during a visit to Nunkuwarrin Yunti for another service.
