Nunkuwarrin Yunti provides a care coordination service to support clients to manage their chronic condition. The team includes an Aboriginal Health Practitioner and Aboriginal Health Worker and supports clients to understand their health needs.

Nunkuwarrin Yunti offers a comprehensive care coordination service to assist clients in managing their chronic conditions effectively. Our dedicated Chronic Conditions Management Team, led by a skilled Coordinator, comprises an Aboriginal Health Practitioner and an Aboriginal Health Worker. We strive to empower clients with the knowledge and tools they need to understand their health needs and make informed lifestyle choices to minimize the impact of their chronic diseases on their overall health and well-being.

How We Can Help

  • Develop Personalized Care Plans: We collaborate with clients, their GPs, and visiting specialists/allied health professionals to create individualized wellness care plans tailored to their specific needs and goals.
  • Ongoing Support and Guidance: We provide ongoing support and guidance to our clients, regularly checking in on their progress, addressing any concerns they may have, and assisting with medication management and access to pharmacy support.
  • Comprehensive Health Assessments: We offer general health checks and immunizations to ensure our clients' overall health is well-maintained.
  • Access to Other Nunkuwarrin Yunti Services: We facilitate access to other essential services within Nunkuwarrin Yunti, including Harm Minimization (drugs and alcohol), Tackling Tobacco, psychology and counseling, and the Health Lifestyle program.
  • Specialist Referrals and Follow-up: We arrange appointments and ensure follow-up with visiting specialists (renal, ENT, endocrinology, infectious diseases) and allied health professionals (physio, diabetes educator, podiatrist, psychologists) to support clients in maintaining healthy lifestyles.
  • Targeted Support for Aboriginal People: Our Aboriginal Health Practitioner provides specialized support services for Aboriginal people at risk of or living with blood-borne viral (BBV) illnesses (Hepatitis B, Hepatitis C, HIV/AIDS) and sexually transmitted infections (STIs), as well as their families.
  • Bulk Billing and Affordable Care: All services are bulk billed, and we strive to ensure that any external service providers our clients are referred to also offer bulk billing options.
